In the most important health insurance study ever conducted researchers at the RAND Corporation devised all experiment to address two key questions in health care financing: how much more medical care will people use if it is provided free of charge, and what are the consequences for their health? For three- or five-year periods the experiment measured both use and health outcomes in populations carefully selected to be representative of both urban and rural regions throughout the United States. Participants were enrolled in a range of insurance plans requiring different levels of copayment for medical care, from zero to 95 percent. The researchers found that in plans that reimbursed a higher proportion of the bill, patients used substantially more services - indeed, those who paid nothing used 40 percent more services than those required to pay a high deductible - but the effect on the health of the average person was negligible. In addition, participants who were assigned at random to a well-established health maintenance organization used hospitals substantially less than those in the fee-for-service system, again with no measurable effect on the health of the average person. This book collects in one place for the first time results previously dispersed through many journals over many years. Drawing comprehensive, coherent conclusions from an immense amount of data, it is destined to be a classic work serving as an invaluable reference for all those concerned with health care policy - health service researchers, policymakers in both the public and the private sectors, and students.

For Cuba's supporters, health is the most commonly cited evidence of the socialist system's success. Even critics often concede that this is the country's saving grace. Cuba's health statistics are indeed extraordinary. This small island outperforms virtually all of its neighboring countries and all countries of the same level of economic development. Some of its health statistics rival wealthy industrialized countries. Moreover, these health outcomes have resulted against all odds. Setting out to unravel this puzzle, the author finds that Cuba possesses an unusually high level of popular participation and cooperation in the implementation of health policy. This has been achieved with the help of a longstanding government that prioritizes public health, and has enough political influence to compel the rest of the community to do the same. On the other hand, popular participation in decision-making regarding health policy is minimal, which contrasts with the image of popular participation often promoted. Political elites design and impose health policy, allowing little room for other health sector groups to meaningfully contribute to or protest official decisions. This is a problem because aspects of health care that are important to those who use the system or work within it can be neglected if they do not fit within official priorities. The author remains, overall, supportive of health achievement in Cuba. The country's preventive arrangements, its collective prioritization of key health areas, the improvements in public access to health services through the expansion of health facilities and the provision of free universal care are among the accomplishments that set it apart. The sustainability and progress of these achievements, however, must involve open recognition and public discussion of weaker aspects of the health system.

Faced with a public that has preferred to avoid confronting the certainty of fatality or the probabilities of catastrophe, insurance promoters have had to create a demand for their products, first, by persuading the public to see the world as ruled less by divine judgments and more by statistical patterns, and second, by proclaiming a moral imperative of hedging against death and disaster by the prudential recourse to insurance. The essays presented here examine the history of insurance as a process of negotiation between the embedded social, legal, and cultural norms out of which the practice of insurance grew, and the new arrangements and sensibilities that insurance itself helped bring into being. Today, insurance is a global economic colossus and a fixture in the developed countries of the world. But neither the financial clout of the insurance industry nor its ubiquity conveys the full measure of its social and political influence. The insurance industry has in fact become a primary agent of discipline and control over public and private behaviours by imposing upon them the criterion of insurability. By tracing the boundaries of acceptable (and compensated) from unacceptable (and uncompensated) risk, insurers directly or indirectly govern people, products, and markets, and by this process become one of the most powerful and pervasive agents of social and economic control.
